When filling out this questionnaire, please keep a few things in mind:

- Each question corresponds to an Indicator of the Criteria and each has a different score. However, to ensure objectivity when answering, these scores will not be displayed on the questionnaire. Instead, you will receive the final overall score via the email you registered with

- When answering, if you choose “Yes”, you will need to select one of the proofs that your business avails of, in the box "Proof, if yes"

- The questionnaire contains many questions, so you likely won’t be able to answer them in one go. To facilitate filling in the questionnaire, you can click the white button (Save) at the end of each page, thereby saving the answers you already provided and enabling to continue the next time(s), before clicking the final submit button when all questions have been replied to.

- After clicking the Submit button, you will receive an automatically produced email with your Provisional Score. The Final Score will be announced by the Assessing Body, after an official review has been undertaken.
